Asthma Action Plans

Asthma Action plans are designed and personalised to give you the best control of your asthma. 

They help to remind you:

  1. Which medications to take every day to reduce your risks of having asthma attacks
  2. What to do if your asthma is getting worse, from day to day activities to asthma attacks

Please ensure you have you Action Plan updated when you see your GP, Asthma Practice Nurse, the Darent Valley Hospital Children's team or our Paediatric Asthma Service.

If you don't have one, please ask for one! You can download the one we use at Darent Valley Hospital below, or use one provided by asthma.org.uk

When we see you we will provide you with an updated copy too and update your records. Please make extra copies and share them with your child's nursery / school, put them on the fridge, save a copy on your phone, and keep them updated!
